You need:
Cardstock
Paper Napkin (top layer)
Baking Paper
Cling film
Iron (no steam!)
Patience 
When you have cut out the serviette motif, remove the uppermost printed coat carefully (there are usually at least 3 layers) and put it on the cling film. Heat it up until the Paper Napkin will merge with the cling film. Take your time!!!
Your sandwich should look like that:
